The Role

As the first point of contact for our patients and clinicians, you play a vital role in our care network. You will bridge the gap between patient needs and provider schedules, ensuring seamless operations and compassionate service delivery.

This isn't just an administrative job; it's a critical coordination role that ensures our patients receive the timely care they deserve in the comfort of their home.

WHAT YOU'LL DO

Key Responsibilities

Process Referrals

Manage incoming patient referrals efficiently, ensuring all initial data is captured accurately.

Verify Insurance

Confirm eligibility and benefits coverage for Medicare, Medicaid, and private insurance plans.

Coordinate Schedules

Match clinicians with patients based on geography, skill set, and availability

Clinician Support

Serve as the primary liaison for field staff regarding schedule changes and patient updates

Data Entry

Maintain accurate electronic medical records (EMR) in compliance with state and federal regulations.

Team Collaboration

Participate in weekly case conferences and coordinate with the Director of Nursing

Required Qualifications

High School Diploma or equivalent required.
Billingual (English/Spanish) is mandatory for this role.
Minimum 1 year of experience in a medical office or healthcare setting.
Strong computer literacy (Microsoft Office, Email, Web, Portals).
Excellent verbal and written communication skills.

Preferred Qualifications

Experience with WellSky (Kinnser) software is highly desirable.
Prior Home Health specific intake/scheduling experience.
Understanding of Medicare/Medicaid guidelines.
Associate's Degree in Healthcare Administration or related field.
